Expression of the Genes Thymidylate Synthase [TS], Dihydropyrimidine Dehydrogenase [DPD], Thymidine Phosphorylase [TP] und Topoisomerase I [TOPOI] as Prognostic Markers for the Chemotherapy of the colorectal carcinoma

Recently, it was suggested to test several prognostic markers in order to improve the chemotherapy of the colorectal carcinoma. For most of the markers Oncoscreen developed a test. The clinical relevance of these markers has not been validated and they have been still in debate within clinical studies.

Expression of the Thymidylate Synthase, Thymidine Phosphorylase und Dihydropyrimidine Dehydrogenase in the Colorectal Tumor as Prognostic Marker for the Response to 5-Flourouracil

Oncoscreen established a technology to quantify the gene expression of the Thymidylate Synthase (TS), Thymidine Phosphorylase (TP) und Dihydropyrimidine Dehydrogenase (DPD) in the colorectal tumor and normal tissue. We offer this test as a service to interested physicians and patients as well as academic and industrial institutions. The tissue can be sent as deep frozen tissue (freshly frozen), as paraffin embedded or as buffer conserved tissue. Oncoscreen developed for each form of conservation an optimized isolation procedure.

The three mentioned enzymes influence the action of the chemotherapeutic drug 5-Fluorouracil (5-FU). The Thymidylate Synthase is the ultimate target of the 5-FU; Thymidine Phosphorylase is involved in the metabolism of the 5-FU, and Dihydropyrimidine Dehydrogenase is the rate limiting enzyme in the catabolism of the 5-FU.

For the adjuvant and the palliative therapy the results of the expression analysis of the three genes must be differently interpreted.

For the palliative 5-FU therapy a lower expression of all three genes correlates with a better prognosis of the response of the tumor to the therapy. If the tumor show a higher expression of all three genes the patient should get a different chemotherapy.

On the other hand, for the adjuvant therapy a higher TS expression correlates with lower recrudescence probability.

Genotyping of the Thymidylate Synthase Promotor as a Prognostic Marker for the Response to the 5-Flourouracil Treatment

The enzyme Thymidylate Synthase (TS) is inhibited by 5- Flourouracil (5-FU). In consequence, the Thymidine amount which is important for the DNA synthesis decreases and the rapidly growing tumor cells die.

he amount of TS enzyme is depended on the activity of the TS gene which, in turn, is depended on the TS promotor status. Two specific alleles of the TS promotor are known one of which decreases the activity of the TS gene. Several clinical studies showed that the tumors of patients with the decreased TS activity responded better to the 5-FU drug.

Expression of Thymidine Phosphorylase as a Prognostic Marker for the Response to Capecitabin/Xeloda

Capecitabin (Xeloda) is a so-called pro drug which is rapidly absorbed by the gastrointestinal tract and is metabolized into 5-Flourouracil (5-FU) by the enzyme Thymidine Phosprorylase (TP) within the tumors. Tumors with a high expression of the TP gene are good candidates for the treatment with Capecitabin. The expression level of TP in the tumor can be analyzed by a quantitative RT-PCR technology which Oncoscreen developed.

Expression of the Topoisomerase I as a Prognostic Marker for the Response to Irinotecan (CPT-11, Campto)

The chemotherapeutic drug Irinotecan (CPT-11, Campto) inhibits the Topoisomerase I (TOPO I). In consequence, rapidly growing cells like tumor cells are not able to finish the DNA replication and, therefore, die. There are numerous scientific reports which show that a high expression of the TOPO I gene correlates with a higher response to TOPO I. The expression level of TOPO I in the tumor can be analyzed by a quantitative RT-PCR technology which Oncoscreen has developed.
